Fever and Rash in a Child: New England Medical Journal Posts Diagnostic Puzzle

The New England Journal of Medicine has shared a real-world case involving a five-year-old boy with fever and rash, inviting clinicians and the public to test their diagnostic thinking through an interactive tool called Doctordle.

The post came from the NEJM’s official account and points to a case currently live on Doctordle, a diagnostic guessing game modelled loosely on the Wordle format. The case — a young boy arriving at an emergency department (ED) with two of the most common symptoms that prompt parents to seek urgent help — is drawn directly from NEJM’s clinical archive.

Fever and rash together in a child can signal anything from a mild viral illness to conditions that need rapid assessment. The NHS advises parents to contact 111 or seek urgent care if a child has a high temperature alongside a rash that does not fade when a glass is pressed against it — a check for meningococcal disease (a bacterial infection affecting the blood and brain lining). That glass test, sometimes called the tumbler test, is one of the clearest at-home indicators that emergency care is needed immediately.
