A new review article in the New England Journal of Medicine (NEJM) outlines the factors clinicians weigh when selecting an antiretroviral therapy (ART) regimen — the combination of medicines used to suppress HIV in the body. According to the journal, the decision rests on several considerations: how well the drugs work, how easy they are to take, their side effects, how they interact with other medicines, and whether the patient can access them.
The NEJM states that two- or three-drug regimens taken once daily by mouth are the standard starting point for most patients. That simplicity matters — a once-daily pill is easier to stick to than a complicated schedule, and consistency is central to how ART keeps HIV under control.
Patient factors also shape the choice. Coexisting conditions — other health problems a person already has — can rule out certain drugs or make others preferable. Drug interactions are a particular consideration, since people living with HIV may take medicines for other conditions at the same time.
