Two US infectious disease experts argue in the New England Journal of Medicine that observational studies deserve more weight in assessing how well vaccines work.
A new perspective piece published in the New England Journal of Medicine (NEJM) makes the case that a widely-used type of vaccine research is being unfairly sidelined. Arnold S. Monto, MD, and Helen Y. Chu, MD, MPH, argue that well-designed observational studies — research that tracks real-world outcomes in people rather than assigning them to controlled trial groups — are critical to advancing understanding of how vaccines perform in practice. But, they say, such studies are routinely dismissed as not being methodologically rigorous enough to inform policy decisions.
The debate matters because observational studies are often the only practical way to assess vaccine effectiveness across large, diverse populations over time. Randomised controlled trials, while considered the gold standard, can’t always capture how a vaccine behaves across different age groups, health conditions, or emerging variants once it’s out in the real world.
The NEJM is one of the most cited medical journals globally. A perspective piece in its pages doesn’t set clinical policy, but it does signal where expert debate is heading — and this one lands at a time when public trust in vaccine science remains a live issue in the UK and beyond.
